Pounds-to-kilograms conversion for dosing Weight (kg) = Weight (lb) ÷ 2.2
Weight-based orders are written in mg/kg, so a weight given in pounds must be converted to kilograms first. Divide the pounds by 2.2 (the teaching convention 1 kg = 2.2 lb).
