How do I calculate a dose by weight?
Convert the weight to kilograms if it was given in pounds (divide by 2.2), then multiply the kilograms by the ordered mg/kg. For a 22 lb child at 10 mg/kg: 22 / 2.2 = 10 kg, and 10 kg x 10 mg/kg = 100 mg.
Why convert pounds to kilograms first?
Weight-based orders are written in mg/kg, so the weight has to be in kilograms before it is multiplied. Using pounds instead of kilograms gives a dose 2.2 times too large, which is one of the most common paediatric dosing errors.
What does mg/kg/day divided into 4 doses mean?
The ordered figure is the total for the whole day, not one dose. Multiply the weight by the mg/kg/day to get the daily total, then divide that total by the number of doses to get each individual dose. Tick the divided-dose box to have the calculator show both steps.
What is the safe maximum field for?
